Reflective Account on Patient Care: Improving Leadership and Decision-Making Skills

I have critically examined a case of patient treatment I gave during my senior year of study in this reflection. The episode of care involved a patient who had poor food and fluid intake and was pacing up and down the lounge. During my interaction with the patient, I encouraged her to eat and supported her in feeding herself while also engaging her in conversation to help her feel more comfortable. I also noticed that she was on blood glucose monitoring, offered her breakfast and checked her blood glucose levels. After breakfast, the patient complained of feeling sick and had neck pain, which I suspected was due to the antipsychotic medication's extrapyramidal side effects (Balpande, Umate and Waghmare, 2021). I informed the duty nurse in charge of the changes I observed and took the patient to her room to lie on the bed and wait for the doctor's assessment.

As a healthcare professional, my adherence to the local and national guidelines for patient care was a top priority during my recent episode of care for a patient with several health challenges. This included demonstrating my decision-making and leadership skills in responding to the patient's specific needs. One key aspect of my response was promptly notifying the duty coordinator of the changes observed in the patient (Vázquez?Calatayud et al. 2022). This ensured that the appropriate measures could be taken to address the patient's poor food and fluid intake, pacing behaviour and physical health problems. This action was important because it ensured that prompt medical attention was given to the patient and her condition did not deteriorate further.

My prompt action in notifying the duty coordinator allowed the patient to receive the appropriate care and treatment she needed, which could have prevented further complications. I also recognized the importance of providing emotional support and comfort to the patient during her illness, as this can be a critical factor in the patient's overall recovery. I was aware that the patient may have been anxious and afraid due to her physical symptoms and general condition, so I made an effort to provide emotional support to her. I offered her a sick bowl, removed one layer of clothes to prevent her from sweating and supported her in her incontinence needs. These actions helped to alleviate the patient's anxiety and fears and made her feel more comfortable and at ease (Zhang et al. 2020). In addition to these actions, I analysed the roles and responsibilities of other members of the multidisciplinary team involved in the delivery of care. This allowed me to work collaboratively with my colleagues to provide comprehensive care to the patient, with each team member contributing their unique skills and expertise to the care process.

As a healthcare professional, I am fully committed to ensuring safe and effective patient care. One of the ways I do this is by adhering to the NMC Code, which sets out the professional standards and behaviour expected of nurses and midwives in the UK (Ross et al. 2021). I understand that the NMC Code is a vital tool for healthcare professionals like myself to ensure that we provide the highest standards of care to our patients. It highlights the importance of working collaboratively with other healthcare professionals, promoting patient safety and well-being and maintaining confidentiality and privacy. I can ensure that the patient's needs are met and that they receive the best possible care. I understand that healthcare is a team effort and by communicating and collaborating effectively, we can provide a holistic and comprehensive approach to patient care, by working collaboratively with other healthcare professionals. This involves communicating with the duty nurse in charge and the duty doctor to ensure that the patient received the appropriate care and treatment (Akku? et al. 2022). I also recognized the impact of healthcare policies on the delivery and coordination of care and followed these policies to provide safe, effective care to the patient. Moreover, the NMC Code emphasises the importance of maintaining patient confidentiality and privacy. As a healthcare professional, I understand the sensitive nature of patient information and I take every precaution to ensure that patient data is kept confidential and secure. Overall, following the NMC Code is an essential aspect of my role as a healthcare professional. It helps me provide safe and effective patient care and ensures that I maintain the highest standards of professional behaviour and ethics.

Throughout my work as a healthcare professional, I have always recognized the importance of effective interprofessional working. This was particularly evident during an episode of care for a patient with poor food and fluid intake, pacing behaviour and physical health problems. In order to provide the best possible care to the patient, I collaborated with other healthcare professionals, including doctors, nurses and support staff (Held et al. 2019). Working collaboratively, we were able to assess the patient's physical and mental health, as well as identify the appropriate care and treatment required. I ensured that all team members were fully aware of the patient's condition and were all working towards a common goal of providing the best possible patient care. Through effective interprofessional working, we were able to provide comprehensive care to the patient and ensure that all of her needs were met (Babaii, Mohammadi and Sadooghiasl, 2021). This included providing emotional support and comfort to the patient during her illness, as well as ensuring that her food and fluid intake was sufficient to meet her needs. Overall, I strongly believe that effective interprofessional working is essential in order to provide the best possible care to patients. We can ensure that patients receive comprehensive care that is tailored to their specific needs, by working collaboratively with other healthcare professionals.

During this episode of care, I demonstrated effective interprofessional working by collaborating with other healthcare professionals to provide the best possible care to the patient. I communicated with the duty nurse in charge to inform her of the changes I observed in the patient's condition and worked together with the duty doctor to assess the patient's physical and mental health. I was able to provide the patient with the appropriate care and treatment, by involving other members of the team. In terms of my own personal and professional development, I recognize that there is always room for improvement. Although I believe that I made good decisions during this episode of care, I also acknowledge that there is always more to learn. Moving forward, I intend to continue to improve my skills, knowledge and attitude towards hospital care and management (Kwame and Petrucka, 2021). I recognize the importance of ongoing learning and development and will actively seek out opportunities to enhance my understanding of current guidelines and best practices. As I have begun to work more independently in the provision of care, I have also developed my decision-making and leadership skills (Nymberg et al. 2019). I have developed important decision-making skills by independently assisting patients with personal care, monitoring their blood glucose levels and giving ward round handovers. Moreover, by becoming aware of my boundaries and limitations when working independently, I have learned to recognize when to seek the advice and support of other healthcare professionals.

During the episode of care, I had the opportunity to provide care for a patient with poor food and fluid intake, pacing behaviour and experiencing physical health problems. As a healthcare professional, it was important for me to demonstrate my decision-making and leadership skills by following local and national guidelines for patient care (Bourgeault et al. 2020). I was able to provide safe and effective care for the patient while ensuring that their rights and safety were protected, by adhering to these guidelines. One of the key decisions I made during this episode of care was to quickly notify the duty coordinator of the changes I observed in the patient. This action was important because it ensured that prompt medical attention was given to the patient and her condition did not deteriorate further. My prompt action in notifying the duty coordinator allowed the patient to receive the appropriate care and treatment she needed, which could have prevented further complications.

I was able to demonstrate my ability to work independently and make effective decisions in a healthcare setting, by taking these actions. This experience has further developed my leadership and decision-making skills, which will be valuable in my future practice as a healthcare professional. I believe that it is important for healthcare professionals to constantly seek out opportunities for learning and development and I will continue to do so to further enhance my skills and knowledge. Moreover, effective teamwork is crucial in achieving the best outcomes for patients during this episode of care (Casey et al. 2020). I critically analysed the roles and responsibilities of other members of the multidisciplinary team in the delivery of care. I recognized that each team member has a unique role to play in the care process and by understanding their roles and responsibilities, I was able to work collaboratively with them to ensure that the patient received comprehensive care. This involved communicating with the duty nurse in charge to inform her of the changes I observed in the patient's condition and working together with the duty doctor to assess the patient's physical and mental health.

Additionally, I examined the impact of healthcare policies on the delivery and coordination of care. I understood that policies and guidelines are in place to ensure that patients receive consistent, high-quality care and that they are followed by all healthcare professionals. This experience has taught me the importance of working collaboratively with other healthcare professionals and seeking out their support when necessary.

During this period of treatment, I was able to push myself and grow in many different ways. Perhaps the most important of these is developing my decision-making and leadership skills. As a healthcare professional, these skills are essential to providing effective treatment and ensuring the safety of my patients. I also gained a deeper appreciation for the roles and responsibilities of other members of a multidisciplinary team. Reflective account-

Reflective account 1:

During the clinical placement in the first episode of care, I was sent to take care of a patient who is facing issues related to the side effects of antipsychotic medications. She was not taking food due to the medication. I realised that she was not able to sit in a chair as well and she was required to lie on the bed. I have completed this overall care assessment based on the care act 2014 which helped me to improve my knowledge on societal care. In the second episode of care, I identified that she is also on a mood stabiliser which is affecting her food habits to some extent. The main caregiver and I have collaboratively prepared a plan to investigate the manageable areas.

I have learnt that medication related to mental health problems can affect the food habits of an individual. In that case, I was able to initiate a plan for improving their needs.

During the situation, I have mainly focused on the person-centred approach along with the Holistic approach to minimise the risk and improve the health care for the patient.

As per the code of the Midwifery Council, I have mainly focused on Prioritising people. Additionally preserving safety and promoting professionalism and trust were other significant codes which have been implemented in this section (Casey et al. 2020).

Reflective account 2:

I have taken guidance from my mentor regarding the care policies to maintain transparency in patient care. As my mentor has an overall idea about my previous practices and has directed me to the right way for treating the patient. In that case, I was very anxious regarding my duty during the episode of care before attending the clinic. I have conducted a detailed discussion with my mentor to provide the best treatment. I have focused on my practice style and focused on facilitating a better experience for the patient. It is highly required to analyse the overall scenario for reflecting a better outcome as per the situational maturity (Zhang et al. 2020).

I have learnt the way of reflecting on experience based on the practical evidence. Moreover, I also have good ideas regarding the required preparation for knowledge management.

I have focused on ongoing assessments of learning to improve my personal skills. Moreover, I have also intended to develop skills for my upcoming career.

In this section I have also focused on effective practice, prioritising people and preserving safety according to the code of the Midwifery Council.

Reflective account 3:

I got a significant idea about how an acute patient can acquire a self-harming instinct during the period of long-term admission.

Repeated self-harming activities are very shocking for medical practitioners. The patient 's medical requirements were not met adequately which has continuously affected her mental state and Physical health at the same time. So she tried to keep her distance from people. I have also evaluated the required emotional support for the patient to analyse the stigmatised beliefs. As a result, it would help me to look at diagnosis from an avoiding danger by taking careful, prudent, and organised steps. Additionally, it also can outline required emotional support (Held et al. 2019). I have also faced while investigating the required way for addressing her winter help issues. In that case, I required adequate guidance immediately for providing better treatment.

During the session, I gathered significant knowledge regarding how to treat a mental health patient who is also suffering from physical issues. As the patient was also taking ECG, BM and others along with antipsychotics, in that case, she needs special care for further development.

After this evaluation, I have mainly focused on continuous evaluation to ensure further development for care and patient management. Additionally, I have also intended to minimise stigmatising behaviour.

According to the code of the Midwifery Council, I have focused on prioritising people, preserving safety and promoting professionalism.

Reflective account 4:

I was asked to support patients who are suffering from mental health issues such as depression, anxiety, self-harm and others. During this psychiatric treatment, we have discussed in a collaborative manner to evaluate the required environment for her. We have also analysed the physical issue that the patient is facing regarding food intake and other. Additionally, their behaviour towards a meeting with a stranger has also been analysed to investigate their mental insecurity and discomfort (Ross et al. 2021). Through the section, I was required mental health treatment to provide comfortable treatment and maintain the overall betterment of the patient. During the session, I understood the challenges while treating mental health patients.

After this session, I have learnt about how I can help mental health patients to most their recovery without harming their physical well-being as well.

After that, I would promote more patient-centric activities to develop my future as a caregiver. Additionally, I would also evaluate the acute mental health settings to identify their basic needs. In accordance, I have also prepared a plan for improvement

I have also focused on prioritising people, preserving safety and promoting professionalism.

Reflective account 5:

I have attended a leadership program to identify the required skills for Mental Health Caregivers. I was selected for the scholarship which has helped me to grab 6 months of CPT experience. Through this experience, I have obtained various cubes like effective communication, team building, leadership development and others. I think it would help me to improve my upcoming career. Additionally, I have also identified how to handle mentally disturbed patients with anxiety and depression to provide them with a compoundable environment. In accordance, this experience has also helped minimise workplace issues and maximised problem-solving skills for further development (Nymberg et al. 2019).

Through this CPD experience, I have gathered information regarding the way of improving leadership skills. In accordance, I have also gathered knowledge regarding the way of improving emotional intelligence while treating mental health patients.

Additionally, I have also focused on improving the quality improvement regarding health care treatment. Moreover, I have focused on my personal skill development to maximise my future career.

Moreover, I have also focused on promoting professionalism and trust, preserving safety, practising effectively and prioritising people.
