Introduction
In this paper the case study of Health and Social Care in the Community has been chosen in order to discuss every aspect that the paper wants to discuss about. Therefore this paper is going to discuss several health issues that one community has to suffer very often. In the older community of the UK heart diseases, stroke, lung and liver diseases are very common. In the past few years, a report has shown that the total number of deaths among the people from the age group of 65 to 75 in the whole of England is almost 150000 a year. Among the total number of these deaths almost, 30000 deaths are avoidable and the others were because of these diseases. Another report has been shown that the suicidal cases and road acciudents among the middle aged person were because of violence, mental stress and because of the intoxicated situation. Mental stress is a major issue that the urban people of the United Kingdom have to suffer from, mostly because of their lifestyle. Some chronic diseases such as pneumonia, influenza and other lung diseases are also common among the youngsters for excessive smoking habits and some other reasons like cold and weather.

A study that has been done among the people of the UK Biobank cohort between 502505 people shows that 60.7% of people died because of lower respiratory functioning, 14.20% of people died because of gastrointestinal diseases, genitourinary diseases have killed 5.10% of people. In this context of the research some factors of facilitators have been found and these are various types of laboratory tests, medicational processes and the new implementations in the medical science. The collaboration of these factors is beneficial the matter of the wellbeing of the patients (Adams, 2012). Not just the facilitators but also some barriers have been found in this background. There are different kinds of barriers that negatively affect the wellbeing of the patients. The whole society is being affected by these things.
In order to make people aware of the health care in any community and to motivate them to take care of themselves and to encourage others two ways have been considered and they are collaborative care planning and health coaching. Not just to enlighten them about self care but also to provoke them to take care of the society. A community health not only deals with the wellbeing of some people or the wellbeing of a group but it concerns the whole community and their wellbeing not just health issues but an overall wellbeing. Collaborative care planning is something that deals with the priorities and opinions of a person that has some live experiences about a specific issue. This can be anything such as, career issues, mental health issues, family problems and many more. Health Coaching is something that teaches people about self care, and motivates them to take care properly with appropriate techniques. While talking about the health and social care of a community a singular planning method is not enough to make a whole community healthy. Collaborative care planning is important to make them more careful about the health of the whole society. Therefore these two ways will be more efficient to improve the condition of a whole community. The main body of this essay will discuss the key factors of these two chosen plans and how they were working in favour of improving the whole condition of the community.
Discussion of the case study
Person Centered care that is also known as Patient Centered care, according to the World Health Organization, it is a process that motivates the common people to improve their health condition on their own or take responsibility for their own health rather than taking help from other people. This whole process is concerned about the views, opinions, experiences and preferences of the patients. This main thing is to improve the health condition of the patients based on their personal experiences, this will help the people more efficiently as every patient needs treatment according to their special requirements. Interaction is one of the main keys that creates a bond between the patient and the doctor (Adams, 2012). This process is really helpful in their treatments as this helps the doctor to understand the actual problem and requirements of a patient. This kind of treatment has a very important role in treating the patients so this type of treatment and treatment centre need to be implemented in the various medical grounds.
As the person-centred care basically deals with the main objectives or main requirements of the patients so in this type of medical field the two interventions, collaborative care planning and health coaching, has an important role. This person centered care has some key principles and they were discussed below. The first and most important principle is to show respect and valuing the needs, preferences and values. As this type of treatment is considered as a good relationship of collaboration and respect so in this method the preferences of the patients has to be considered in a broader way. Health coaching in this regard is so valuable and can be connected with this field. The next one is to coordinate and integrate with care. The improvement in coordination and integration in the field of medicine or in every clinical section help to decrease the vulnerability and uncertainty among the patients. When the patients will notice the people from medical background to value their opinions and requirements they will be a bit more confident and secure with their treatment procedure. These two methods can be treated like the intervention of the health coaching.
The next principle is to provide information to the patients. As this process includes the opinions and views of the patients so in this treatment process patients are getting involved and their medical condition has to be discussed with the patients (Neuberger, 2018). In order to make proper decisions the patients even provided some education regarding their health condition and about that particular disease background. This proper knowledge will guide them to make proper decisions. In the recent past pandemic situation when the whole world was facing a lot of health issues that are matters that not just the matter that can be mitigated by the internal person-centered care but also need some external involvement in this context. In this context health coaching and collaborative care planning is important and relevant. The next principle of physical comfort is the key to the proper person centered care facility and the accurate way to make the experience of the patients better. Emotional Support is the most important thing that one patient is facilitated to get in this special method of treatment. Often in the patients the level of confidence is too lowered and they mostly were suffering from mental health issues for their bad physical status. So this emotional support works a lot more helpful for the patients. A health coach not only guides in improving the health condition but they also try to interact with the patients that help understand the patient more prominently.
The involvement of family and friends in their treatment methods also proved a lot helpful for the patients along with the doctors. As the family and friends always notice the patients on a regular basis so they can provide more details about them to the doctors. It will also help the doctor to treat in a more efficient way (Neuberger, 2018). The involvement of the family and friends also help in taking care of the patients in the absence of the doctors. This process is connected with collaborative care planning. In that case primary care and secondary care both are important. In the recent pandemic situation when people were facing a lot of health issues and having major problems that can be mitigated by these two interventions, collaborative care-planning and health coaching. These two interventions include the patient's opinions that must need to be implemented in that particular section where the person centred care method of treatment is implemented.
In the previous sections, two interventions of person centered care, the relevancy of collaborative care planning and the health coaching has been discussed. Collaborative care planning can be used as the key to the health and social care in the community. A community health care deals with every health issue that the community has to face. The diseases that are common among the UK communities are discussed at the introduction section. Those diseases are very important to be cured. According to some research that has been conducted in order to complete this research paper, this collaborative care planning is so important. This paper has been designed not just to discuss health care but also social care. The matter of social care in any community deals with the matters of mental stress, community situation and many other problems that are connected with the whole community or that might affect the whole communal environment. In this context the legislative stations need to be discussed. Here in this essay it also included the benefits of healthily coaching and its priority. In the legislative porting regarding health care some law has been discussed by the health condition.
The intervention of health coaching is a process by which the coach is able to motivate, guide and make the patient more attentive in taking care of himself or herself. The health coach is basically a doctor, teachers, a nutritionist and an exercise trainer. The health coach makes the patient reliable in the context of taking care of the patients. He is basically the cheerleader that the patient follows to improve their health condition and their mental situation. In context of the societal health condition this particular intervention can be used as a communal health by creating a community club. In those clubs some classes have to be organised that provide those health coaching systems (Neuberger, 2018). In those classes the coach should give advice regarding the different types of health issues and their mitigation techniques. In those classes they also discussed the food habit and their issues regarding the main problems.

The collaborative care planning in the health and social care in the community mainly include for the proper planning and preferences that re-included to develop the to lived the experiences and also help the patient to gain some knowledge about the diseases that have been taken place this makes them confident as well as it also helps in becoming activity as well as it also helps to develop the integration of health care this also help to evaluate the multiple effective treatments to the evaluated the skilled full strategies to develop the strategies. Here in this portion, it will mainly be discussed about the health social care in the company that is befitting the health and social care in the company that mainly provide the health care support to the nursing homes as well as to the foster caring and it also helps to develop the social care support as well as it also helps in the maintaining the working relationship and also provide the first priority to the friends and family that also help to maintain the normal lives that the people wanted to live that is because it is so important for the health social care in the community. In the whole discussion part there are some legislative discussions that have also been concluded. Here in this portation, it will discuss the health and social care in the community that helps to maintain the health properly which helps the individual to maintain the proper health of the whole community. There are some functions that used to maintain the health and solved the health problems of the people that all are discussed above.
